Volunteer Recognition Form
Doing Good celebrates those who do good! We share the story and passion of volunteers to educate and (hopefully) inspire others while recognizing and celebrating well-deserving volunteers. This also provides each agency served by the volunteers free visibility and awareness.
You may nominate another or yourself. The request is to complete as much of this form as possible. If you nominate another, he/she will be asked to complete it for themselves.
All nominations:
- Can be one person or two who serve together.
- All should live in the United States.
- A diversity of ages, ethnicities, races, religions, abilities and disabilities is encouraged.
